The Complete DQ File Checklist
for Trucking Companies

This guide is brought to you by Driver Pipe — Applicant Tracking, Recruitment Tools, and DQ File Management for smaller carriers.

The information is provided for informational purposes only and does not constitute legal advice. Motor carriers are responsible for complying with applicable FMCSA regulations.

Understanding Driver Qualification and Pre-Employment Compliance Requirements

If you operate a trucking company in the U.S., you are required to maintain a Driver Qualification (DQ) file for every driver who operates a commercial motor vehicle (CMV) on your behalf under 49 CFR Part 391.

CDL drivers are also subject to additional pre-employment compliance requirements under 49 CFR Part 382 and Part 40.

These records help demonstrate that each driver is properly qualified, vetted, and authorized to operate a commercial motor vehicle.

Required Driver Qualification Records

Required DQ File Records 49 CFR Part 391
  • DOT-Compliant Employment Application
  • Safety Performance History Investigation
  • Initial Motor Vehicle Record (MVR)
  • Road Test Certificate or Valid CDL Copy
  • Annual MVR and Driving Record Review

Pre-Employment Compliance Requirements 49 CFR Part 382 and Part 40
  • Pre-Employment Drug Test
  • Pre-Employment Clearinghouse Query

These are required before a driver operates a CMV, but should be maintained separately from the DQ file.

If your DQ files or compliance records are incomplete, missing, or outdated, you risk violations, penalties, and compliance issues during audits.

What Must Be Updated Every Year?

Driver compliance is not a one-time setup — it requires ongoing reviews, monitoring, and annual updates.

Annual MVR and Driving Record Review

Obtain a new Motor Vehicle Record (MVR) at least once every 12 months and document that the driver remains qualified.

Annual Clearinghouse Query

Run an annual limited query through the FMCSA Drug & Alcohol Clearinghouse to check whether the driver has any drug or alcohol violations.

Expiration Date Monitoring

Track CDL and Medical Certificate expiration dates to help ensure drivers remain properly licensed and medically qualified.

Common DQ File Violations

  • Expired Medical Certificates — One of the most common compliance issues identified during audits. Most medical certificates are valid for up to 24 months. Set reminders before expiration dates to help prevent drivers from operating with expired medical certification.
  • Missing Previous Employer Inquiries — Previous employer safety performance investigations must be completed within 30 days of the driver's hire date. Document all contact attempts, including the date, method, and outcome.
  • Missing Annual MVR Reviews — Annual MVR reviews must be completed every 12 months, not just during the hiring process.
  • Incomplete Employment Applications — Missing work history, unexplained employment gaps, or missing signatures are common compliance issues.

How Driver Pipe Helps You Stay Organized and Up to Date

Driver Pipe helps trucking companies centralize DQ Files in one system, monitor compliance requirements, and maintain a clear status overview for each driver.

Retention: DQ files must be kept up to date and maintained for as long as the driver works with your company, plus three years after the driver leaves.
